The name of the answer, and how it will look on reports.

Display Text

What text will appear next to the check box or radio button.

Answer Weight

This field is used for weighted forms and is very important to fill out! If you do not want the answer to effect the weight, set it to -1. 
 

Intro Text

Specialized text that can appear above the form answer.
 

Display Order

This number controls the order of the answers to the question.
 

Other

When set to 'Yes', will provide a text box next to the check box or radio button.

Required

Sets whether the answer must be selected for the form to be marked as complete. Mainly used for confirmation questions (' I have read the agreement...')
 

Status

To remove an Answer, set the status to 'inactive'.
 

Note

Instructional text that appears under the Answer.
 

Description

If this is filled out, the form name will become a hyperlink. When clicked, the form answer will open a pop-up with the description text.
